TO MAKE AN APPOINTMENT
Click on the calendar to select a date/time that is best for you. Enter the requested information into the page and hit submit. A confirmation number will be provided confirming your appointment with us.

APPOINTMENT INSTRUCTIONS
A scheduled appointment takes about 20 minutes per session. A session may consist of (1) CAC ID or (1) Dependent ID cards. If you fail to schedule multiple appointments for multiple IDs, you will have to reschedule the remaining IDs for a later date.

DOCUMENTATION REQUIRED TO ESTABLISH IDENTITY
All customers age 18 and older must present (2) forms of identity documents. One MUST be a valid state, federal or local government entities/agencies issued picture ID.

UPDATE ADDRESS & PHONE NUMBER IN DEERS
For customers who have already been issued a CAC card, an appointment is not required to update your address and phone number in DEERS. Just visit this website: www.milconnect.dmdc.mil

CAC RESET: INVALID PIN/BLOCKED/LOCKED CAC ONLY
No appointment is needed for a CAC reset ONLY if you received an error stating you have been "BLOCKED" or "LOCKED" or you have an "INVALID PIN". This action will be conducted as a Walk-In service and again scheduled appointments are seen first.

LOST OR STOLEN CAC/ID CARDS
All ID Cards are the property of the U.S. Government and holders are required to safeguard their ID Card/CAC at all times. All customers must have a lost/stolen memorandum from their unit or from Base Det. STL and must also present two valid forms of ID.

POWER OF ATTORNEY
If your Military Sponsor is DEPLOYED or on TDY orders and he/she is unable to accompany you for your appointment, a valid "General" or "Special" Power of Attorney will allow you to conduct business on your sponsor's behalf.

JUST GOT MARRIED? INITIAL ENROLLMENT OF SPOUSE
When adding your spouse to DEERS, the sponsor must be present or the new spouse must have a valid POA with the following documents. Valid state, federal or local government entities/agencies issued photo ID and also;

ENROLLMENT OF SPOUSE CONTINUED
Original/certified copy of marriage certificate, birth certificate and social security card/verification of social security number (Memo from SSA Office with social security number printed).

ENROLLMENT OF SPOUSE, DUAL MILITARY
If your spouse is also in the military, both military personnel must be enrolled and listed in each others DEERS profile.

SPOUSE, FOREIGN NATIONALS
If your spouse is a foreign national they must provide an-original marriage certificate, valid passport and birth certificate. All documents need to be certified and translated into English.

DEPENDENT ID CARDS
The sponsor must be present to issue dependent ID Cards. If not available, the dependent must have a DD Form 1172/2 Application signed by their sponsor witnessed by a Verifying Official or Notary Signature and Seal or a POA.

For CAC holders, you can now fill out & sign the DD Form 1172-2 online which will store in DEERS. Please visit this new service at https://www.dmdc.osd.mil/self_service

ADDING NEWBORN/CHILDREN TO DEERS
Sponsor must have the following documents for the child: Birth Certificate/Certificate of Live Birth authenticated by the attending physician & social security card (when available). If a stepchild is being added, the marriage certificate is also needed.

ADDING NEWBORN/CHILDREN TO DEERS CONTINUED
For a stepchild, we will also need the divorce decree from the previous marriage, or a court-verified custody agreement. If the new spouse does not have written proof of primary custody, the child can not be added under the new sponsors profile.

FULL TIME COLLEGE STUDENTS
For eligible 21-23 yr old full-time college students, a letter from the school Registrar with an anticipated graduation date and full time eligibility statement is required as well as a signed DD Form 1172/2.

ENROLLING DEPENDENT WARD
You will need to provide a PPC(RAS) letter of approval, Court Document placing the child in the members household, Birth Certificate and Social Security card. Please note that all documents must be translated into English and be original/certified copy.

CHILDREN BORN OUT OF WEDLOCK
Female sponsors: Birth Certificate and social security card; Male sponsors: Court order establishing paternity or a State Voluntary Acknowledgement of Paternity Form, Birth Certificate, SSN Card.

INCAPACITATED CHILDREN OVER AGE 21
The Military sponsor's parent service must process initial and renewal dependency applications for all incapacitated children as cross-servicing is not authorized.

PARENT ENROLLMENT
For a Secondary Dependent Parent in Law, you will need to provide a PPC(RAS) letter of approval, Service Members SPOUSE Birth Certificate, Social Security Card & a valid Photo ID.

CIVILIANS DEPLOYING OVERSEAS
Bring in two forms of unexpired ID, issued by state, federal or local government entities or agencies; and a completed DD Form 2365. Please make sure it’s properly filled out or they may be rejected.

CONTRACTORS DEPLOYING OVERSEAS
Bring in two forms of unexpired IDs, issued by state, federal or local government entities or agencies; SOFA agreement; and SPOT LOA. Please make sure the forms are properly filled out or they may be rejected.
