Calendar Details

IDENTIFICATION NEEDED
Customers over 18 need two VALID(not expired)forms of identification. State/government PHOTO identification is required. Common forms are driver's license, state issued ID, DEERS military ID. Alternate forms: Passport, VA card, SS Card, Birth Cert.

INCLEMENT WEATHER
During the winter months, please be aware appointments may be cancelled for adverse weather conditions. Please note we follow the Traverse City Area Public Schools and we are closed when they are. We will contact you to reschedule.

REQUIREMENT OF THE SPONSOR
The sponsor must be with you if you are a dependent. If he/she is unable to come at your scheduled time, please have a power of attorney or a notarized dd1172 http://www.cac.mil/assets/pdfs/dd1172-2.pdf. The notarized signature is good for 90 days.

INSTRUCTIONS
Each session is 15 minutes. A session may consist of (1) CAC card, (1) newly retired/newly disabled card, or up to (2) renewal cards.

ACTIVE DUTY/RESERVIST
If you are on active duty or a drilling reservist, on top of your two forms of ID, please bring in your new enlistment contract (if applicable) and/or your advancement/promotion certificate (if applicable).

IMPORTANT INFORMATION
Please be advised that dependents are eligible to carry a card if they have reached their 10th birthday and ID cards can only be renewed WITHIN 30 DAYS PRIOR TO ITS EXPIRATION DATE or anytime thereafter.

SPECIAL FORMS OF IDENTIFICATION (ADDING A DEP)
ADDING A DEPENDENT: Please bring the birth certificate or hospital proof of birth, social security card, marriage certificate, court paperwork (if dep is a ward) and/or adoption paperwork (if dep is adopted).

SPECIAL FORMS OF IDENTIFICATION (21+ STUDENT)
21+ YOA STUDENT: On top of the two forms of ID, please bring a letter from the college registrar office stating its an accredited college, the student is full time and his/her estimated graduation date.

SPECIAL FORMS OF IDENTIFICATION (65+ MEDICARE)
ADDING MEDICARE (65+ YOA): On top of your two forms of ID, please bring in your medicare card for system validation.

SPECIAL FORMS OF IDENTIFICATION (DEC SPONSOR)
RECENTLY DECEASED SPONSOR: On top of your two forms of ID, please bring in the death certificate.

SPECIAL FORMS OF IDENTIFICATION (DIVORCE)
ENTERING A DIVORCE: Please bring in the divorce certificate. Please be advised that this can only be done by the sponsor. If you, as the dependent, think you are eligible for your own benefits, please call Tricare at 1-800-874-2273.

SPECIAL FORMS OF IDENTIFICATION (RESERVE - AD)
RESERVISTS GOING ON ACTIVE DUTY: On top of your two forms of ID, please bring your active duty orders. For your appointment, you must be in uniform.

SPECIAL FORMS OF IDENTIFICATION (RET RESERVIST)
RETIRED RESERVIST REACHING AGE 60: On top of your two forms of ID, please bring in your letter stating that you are eligible for retirement pay.

SPECIAL FORMS OF IDENTIFICATION (RECENTLY RETIRED)
RECENTLY RETIRED: On top of your two forms of ID, please bring in your dd214 and retirement certificate.

SPECIAL FORMS OF IDENTIFICATION (100% DISABLED)
100% DISABILITY-VA: On top of your two forms of ID, please bring your paperwork from the VA stating your 100% disability start date, most recent dd214, birth cert, social security card.

SPECIAL FORMS OF IDENTIFICATION (INCAPACITATED)
INCAPACITATED DEPENDENT UNDER 21YOA: If dependent is under 21YOA, please bring the doctor’s statement and the approved dependency determination from sponsors branch of service. If dependent is over 21YOA, please see below.

SPECIAL FORMS OF IDENTIFICATION (INCAPACITATED)
INCAPACITATED DEPENDENT OVER 21YOA: Please bring the doctor’s statement, approved dependency determination from sponsors branch of service and dd137-5 with its required documents.

LOST OR STOLEN CAC/ID CARDS
In order to receive a new card after its lost or stolen, you must have a valid police report & memo from your command (memo for active duty & reservists only). The two forms of ID are still required.

RECENTLY RETIRED CIVILIAN
There are a few steps for you to take before we can issue you a card. Please begin by calling the Civilian Benefits Info Line at 207-606-1800.

TWIC CARDS
We do not issue TWIC cards at this office. There is an office in Traverse City and it can be reached at 1-866-347-8942.
