ON WEDNESDAY, 18 DECEMBER 2019, THE ENTIRE MPD, WHICH INCLUDES THE ID CARD/DEERS ENROLLMENT OFFICE, WILL CLOSE AT 1130 AM FOR THE REMAINDER OF THE DAY. LAST CUSTOMER(S) SEEN AT 1100 AM. OPENS ON MONDAY, 23 DEC, 0730 AM.

HANDICAP ACCESS RAMP/ENTRY TO BLDG 3401
Additional Handicap parking with a ramp is located in the back, south side, of the building or through the ACS entrance (back center).

WELCOME TO ID CARDS/DEERS ENROLLMENT
Please refer to the information listed below for your appointment or as a walk-in customer. One Person per Appointment Time Slot. Dependents who arrive without their Sponsor will need a valid Power of Attorney or current DD1172 to access the system.

Unfortunately, there are days when we have computer issues that are beyond our control & we apologize for any inconvenience. When this happens we will try to call our daily appointment customers. You may need to reschedule your appointment.

SELF SERVICE LINK - UPDATES, DD FORM 1172 ETC
https://www.dmdc.osd.mil/self_service/rapids The MPD Actions Office will allow the digitally signed DD1172 from this website for some of your Personnel Actions packets (ask your S1). Update your CAC PIV. You can update your family information too.

MAKE/CANCEL/RESCHEDULE APPOINTMENTS
All customers will need to schedule their own appointment. The Clerks can't schedule them. One Person per Appt Timeslot, no groups please as it impacts other appointments. All BDE Soldiers are required to go to Bldg 3424, rm 19 for all CAC requirements.

APPOINTMENTS HAVE PRIORITY & RECOMMENDED
One Person per Appointment Time, no groups per the timeslot as it will effect the next appointment. Request that you arrive on time as to not effect other appointments & customers waiting to be seen. Walk-ins: see below.

STRYKER BRIGADE CAC OFFICE (ie: HQs, BSB, BEB etc)
Per BDE HQ CAC Office (Bldg 3424, rm 19) all Soldiers assigned to Stryker Brigade need to schedule an appointment with BDE CAC office first for all CAC Reasons, ie promotions etc & Form DD1172 print out only (No Enrollments). POC: 353-2362/2166

STRYKER BDE APPOINTMENTS WITH MPD
Per BDE HQ CAC Office (Bldg 3424, rm 19) all Soldiers assigned to Stryker Brigade must schedule an appointment with MPD for CAC issuing, dependent IDs, enrollments, & updates-(no walkins). POC: 353-2362/2166

WALK-IN CUSTOMERS
You may experience wait times of over 2 hours to be seen depending upon the number of customers already waiting. Walk-in customers may not be seen by lunch time or close of business & asked to return later. BDE Soldiers see BDE CAC info above.

MILITARY APPEARANCE FOR PHOTOS
Service Members need to be in compliance with service affiliated Military Appearance Publication Requirements to have their photo taken. DA3349, per DA PAM 40-502, must have CDR digital signature. All Brigade Soldiers must schedule with the BDE CAC Office

ALL CARDS FOR RENEWAL DUE TO EXPIRATION
90 days prior to the expiration date on the card you may renew your respective Card. Sponsor or spouse with POA or signed/notarized properly completed DD1172 & two valid forms (not expired) of ID are required. POAs must have an expiration date.

ALTERNATE PIN RESET/UNBLOCK CAC LOCATION
OPTION: The Network Enterprise Center (NEC) Bldg 1060 on Gaffney Road (btw Apple Rd and Marks Rd) has two CAC Pin Reset/Unblock Systems that take about 5 mins to get done which is quicker than waiting at the BDE CAC Office or MPD ID Card Office.

AUTHORIZED DOCUMENTATION
Bring with you all the required original or actual notarized copy or certified copy of the documentation for your visit. You may refer to the sections listed below, plus the self service link listed above. Foreign documents must be translated.

PIN RESETS/UNBLOCK
CAC Pin Reset/Unblock is handled on a walk-in basis only. ALL Stryker BDE Soldiers are directed to Bldg 3424 Luzon, rm 19 for service. CAC Pin Resets/unlock can also be taken care of at Bldg 1061 Gaffney Rd (NEC office has two reset/unblock machines)

REAL ID CARD ACT INFORMATION
The Real ID Card Act does NOT affect ID Card/DEERS operations. For identification purposes, we will still accept any (NOT EXPIRED) photo IDs/Driver's Licenses for the states that have been deemed unacceptable, per the Real ID Card Act.

ACCEPTABLE FORMS OF IDENTIFICATION
To see a list of acceptable forms of identification go to http://cac.mil/docs/List_of_Acceptable_Documents.pdf (You will need to copy and paste the URL). All must be original or certified/notorized copies and not expired or copies of copies.

IDENTIFICATION
You must have 2 unexpired forms of ID, including a photo ID, to get a new ID card made. Customers, 18 yrs & older, must provide a valid state or government issued photo ID with legal name. Dependent ID's are not replaced due to their Sponsor's promotion.

POWER OF ATTORNEY (POA) / DD FORM 1172-2
If the Military Sponsor is unable to visit with you, a valid Power of Attorney (w/expire date) will allow you to conduct business on your sponsor's behalf. You may also provide a copy of the notarized & signed DD Form 1172-2 for card to be issued.

LOST/STOLEN/DAMAGED CAC CARD REPLACEMENT
All Soldiers, Civilians and Contractors: must have 2 valid (not expired) forms of ID (one form of ID must be a federal or state issued photo ID) & MP Report. Please refer to http://cac.mil/docs/List_of_Acceptable_Documents.pdf

LOST/STOLEN ID CARDS FOR DEPENDENTS, PART 1
Dependents need 2 forms of ID (one form of ID must be a federal or state issued photo ID). Sponsor must be present or Dependent must have a valid Power of Attorney or DD Form 1172-2 signed by the sponsor & notarized. (see next block)

LOST/STOLEN ID CARDS FOR DEPENDENTS, PART 2
This applies to all customers, including children over 10 years of age. Customers who are 18 years and older must provide at least a state or government issued photo ID. http://cac.mil/docs/List_of_Acceptable_Documents.pdf

ENROLL A SPOUSE TO DEERS (bring proper documents)
Recommend the Sponsor & Spouse must be present & bring the Marriage Certificate, Birth Certificate, Social Security Card with new name or official document from the Social Security Office and two valid photo IDs. One photo ID needs the new name.

ENROLL A SPOUSE TO DEERS (CONT.)
All documents must be originals or notarized/certified copies. If the Sponsor isn't present, the Spouse will need a POA from the Sponsor to enroll. Go to www.dmdc.osd.mil/rsl to locate nearest DEERS/ID Cards Office.

ENROLL NEWBORN(S)/CHILD(REN)
When is Sponsor married to mother at time of birth, proof of live birth can be used to enroll. SSN Card & birth certificate ARE REQUIRED within 90 days of birth and other older Child(ren) will need birth certificate & SSN card.

CHILDREN BORN OUT OF WEDLOCK
Female Sponsors: must bring Child's Birth Certificate and Social Security Card. Male Sponsors: must bring a certified Court Order establishing paternity or a certified State Voluntary Acknowledgement of Paternity Form, Birth Certificate & SSN Card.

FULL TIME COLLEGE STUDENT, including WARDS, PART 1
Must bring an official/stamped & signed letter (original or original certified true copy) from the School Registrar stating they are officially enrolled full time in an accredited college in pursuit of a specified degree with a tentative graduation date.

FULL TIME COLLEGE STUDENT, including WARDS, PART 2
If unable to receive a registrar's letter, go to the National Clearing House website and print out a current dated semester Enrollment Verification (not the Request) with the graduation date. The website is http://www.studentclearinghouse.org

SECONDARY DEPENDENCY WEBSITE INFO/TOOL

DEPENDENT PARENT ENROLLMENT
Provide the DFAS Letter of Approval, provide both the Service Member's & Parent's Birth Certificate; Parent's SSN Card & a valid government issued not-expired photo ID.

DEPENDENT PARENT IN-LAW ENROLLMENT
Provide DFAS Letter of Approval, Spouse's Birth Certificate, marriage certificate, SSN Card & a valid government non-expired photo ID; Parent In-Law's birth certificate, SSN Card, Marriage Certificate and a valid government non-expired photo ID.

DEPENDENT WARD ENROLLMENT
You will need to provide certified Court Order placing the Ward in the sponsor's household for 12 months or more, Ward's Birth Certificate & SSN card & the DFAS secondary dependency approval letter.

CIV DEPLOYING OVERSEAS OR EMERGENCY ESSENTIAL
Must bring 2 current forms of authorized ID, current DD Form 2365 (EE identified), approved DD Form 1614 or DD Form 1610. Forms must be properly filled out or it may be rejected. See your TASS/HR.

CTRS DEPLOYING OVERSEAS OR EMERGENCY ESSENTIAL
Bring two current forms of authorized ID, SPOT LOA, EE Identified, Statement/Direction of Work/Contract for Overseas Assignment. Forms must be current and properly completed or they may be rejected. See your TASS/HR.

TRICARE YOUNG ADULT (TYA)
TYA includes medical and pharmacy benefits, but excludes dental coverage. Individual is to apply for TYA first. Sponsors must verify dependent eligiblity with their TRICARE office. Once verified by TRICARE, an appointment can be made.

RAPIDS SELF SERVICE (RSS)
The RSS allows Sponsors to update their information and verify dependents for DoD ID card reissuance, DD1172, etc. To access the website, Sponsors must have a DoD CAC Card and access to a CAC enabled computer. The RSS website is located towards the top.

DOCUMENTS WRITTEN IN A FOREIGN LANGUAGE
All foreign documents will need to be translated into English and notarized/certified in English and need to be the originals or certified true copy of the original. No copies of copies are authorized to be submitted.

EXPIRED STATE DRIVERS LICENSE (DL)
Any State DL that has expired is not an authorized/valid photo ID for issuing a CAC or Dependent ID card, even if your state allows you to still drive with the DL when presented with a valid military ID.
