ID CARD OFFICE: ROOM 1153
We respect our customer’s privacy by setting appointments and having a waiting room. So that there is no information shared under the Privacy Act of 1974. With the technology age, protecting information is important to us.

APPOINTMENT INSTRUCTIONS
A scheduled appointment timeframe takes about 30 minutes per session. A session may consist of (2) CAC ID or up to (4) Dependent ID cards. Please schedule multiple appointments based on the number of ID cards needed.

TWO FORMS OF IDENTIFICATION
When coming to the Deers office you will need two forms of identification one has to be Photo ID which includes the following, Drivers license, Passport, Social Security Card, Birth Certificate, Federal or State ID, VA ID card, CAC CARD,DOD CARD.

DEPENDENT'S ID'S REQUIREMENTS
The sponser must be present! Needs to sign a DD FORM 1172 the sponors and dependent's need two forms of ID when you come to the ID card office. If the sponser not available, fill out a DD Form 1172-2, sponsor witnessed by a Verifying offical and sealed.

WALK-IN CUSTOMERS
Emergency cases, or walk-in customers requiring same-day service must provide documentation to substantiate the emergency request, i.e. lost CAC/ID Card, CAC unable to read. All emergency requests must be approved by the(SSM)Site Security Manager or VO.

CAC RESET: INVALID PIN/BLOCKED/LOCKED ONLY
No appointment is needed for a CAC reset ONLY if you received an error stating you have been "BLOCKED" or "LOCKED" or you have an "INVALID PIN". This action will be conducted as a Walk-In service.

LOST OR STOLEN CAC/ID CARDS
All ID Cards are the property of the United States Government and holders are required to safeguard their ID Card/CAC at all times. Pursuant to Directive Type Memorandum(DTM) 08-003 requires an individual present documentation from the Provost Marshall.

LOST/STOLEN ID CARDS FOR DEPENDENTS
Dependents: Must have TWO forms of Identification, please see above instructions. Sponsor MUST be present at time of issue, police report is required prior to receiving a new ID card.

POWER OF ATTORNEY
If your Military Sponsor is DEPLOYED or on TDY orders he/she is unable to accompany you for your appointment for any reason. A valid "General" "Special" Power of Attorney will allow you to conduct business on your sponsor's behalf.

JUST GOT MARRIED? INITIAL ENROLLMENT OF SPOUSE
Sponsor must be present or new spouse must have a valid Power of Attorney with the following documents. A valid state or federal government issued photo ID, Original- certified ID, copy of marriage certificate and Social security number.

NAME CHANGE FOR SPOUSES
According to Defense Human Resource Activity(DHRA), the name may only be changed if the spouse provides at least one identity source document listed on the OMB I-9 form such as a Social Security Card or valid Drivers License reflecting the name change.

ADDING NEWBORN/CHILDREN TO DEERS
Sponsor must have the following documents for the child: Birth Certificate/Certificate of Live Birth authenticated by the attending physician & social security card(when available). If a stepchild is being added, marriage certificate is also needed.

FULL TIME COLLEGE STUDENTS
All College students 21 years old must bring a letter from their school registrar's office stating that they are enrolled full time in an accredited college in pursuit of an Associate Degree or higher to have dependent benefits extended to age 23.

CHILDREN BORN OUT OF WEDLOCK
Female Sponsors:Child's Birth Certificate and SSN; Male sponsors:Court order establishing paternity or a State Voluntary Acknowledgement of Paternity Form,Child's Birth Certificate, SSN and legal documents showing paternity was judicially determined.

ENROLLING DEPENDENT WARD
You will need to provide a DFAS letter of approval, Court Document placing the child in the members household, Birth Certificate and Social Security card. Please note that all documents must be translated into English and certified by the courts.

PARENT ENROLLMENT
You will need to pick up an application and "apply for approval through DFAS". Provide the letter of approval at the time of enrollment, Service Members Birth Certificate & a valid Photo ID of your parent along with their Social Security Card.

PARENT-IN-LAW ENROLLMENT
Enrollment process and documents are the same as the Parent Enrollment listed but instead of Sponsor's Birth Certificate you will need SPOUSE'S Birth Certificate and Marriage Certificate. documents must be translated into English and certified by courts.

CIVILIAN EMPLOYEES
If this will be the first CAC card issue with a new position. You will need to provide your SF Form 50, (Notification of Personnel Action) OR a DD Form 1172-2 Application at the time of issuance "along with 2 forms of ID".

CIVILIAN EMPLOYEES(OVERSEAS POSITION)
Provide your completed DD Form 2365 and SF Form 50 annotating that person is assigned to an Overseas Emergency Essential Position, along with your 2 forms of ID.

VETERANS 100% DISABILITY
VA approved for 100% permanent disability you need to bring the following items into the DEERS office. DD214, VA letter confirming 100% disability and 2 forms of ID.
