Hours Of Operation
Our hours of operation are from 0730 to 1600. Those without an appointment are considered Walk-ins, and are seen on a case by case basis and are dependent on availability. Closed for training every second and fourth Thursday of the month until 10:00 HRS.

ACTIVE DUTY SOLDIERS ASSIGNED TO PSDR UNITS
SM's assigned to PSDR Units (1st BCT, 2nd BCT, 3rd BCT,11th ADA, 1AD SUST, DIVARTY and the CAB) are required to report to their supporting S1/BDE for CAC reissuance, or CAC issues. You will NOT be serviced at this site.

EFFECTIVE 1 MAY 2019 WALK-INS FOR ID CARDS SERVICES AT BULDING 1741 WILL ONLY BE DONE ON THURSDAYS. PLEASE USE THE ONLINE APPOINTMENT SYSTEM FOR FASTER SERVICE. ID CARDS MAY BE RENEWED UP TO 90 DAYS PRIOR TO EXPIRATION. PLEASE PLAN ACCORDINGLY.

**PLEASE READ PRIOR TO SCHEDULING APPOINTMENT**
One appointment slot is for one (1) CAC transaction or two (2) non-CAC transactions. For example, one appointment can be used to replace your Spouse's ID (non-CAC) and your Child's ID. Please make as many appointments needed to cover all services needed.

Renew CAC and Dependent ID Cards
Renewal of ID Cards can be completed no earlier than 90 days before the ID card expires.

What type of ID?
Our office ONLY issues ID's for Service Members(including National Guard/Reserve), Retired Service Members and their dependents, Sponsor Appointed Agent ID's, DoD Civilian, DoD CIV Retired, DoD Contractor EMPLOYEE's ONLY.

APPOINTMENT INSTRUCTIONS
A scheduled appointment timeframe takes about 20 minutes per session. A session may consist of (1) CAC ID or up to (2) Non-CAC ID cards (EX:Retiree ID and Dependent ID). Please schedule multiple appointments based on the number of ID cards needed.

**LATE TO YOUR APPOINTMENT**
Please keep your appointment time. If you arrive 5 minutes after your appointment time, you will be considered as a walk-in customer.

GENERAL ID INSTRUCTIONS

UPDATE ADDRESS & PHONE # IN DEERS
Appointment is not required to update your address and phone number in DEERS. Please visit the following website and update your information: https://www.dmdc.osd.mil/milconnect.

CAC RESET: INVALID PIN/BLOCKED/LOCKED ONLY
No appointment is needed for a CAC reset ONLY if you received an error stating you have been "BLOCKED" or "LOCKED" or you have an "INVALID PIN". This action will be conducted as a Walk-In service and again scheduled appointments are seen first.

LOST OR STOLEN CACs
All ID Cards are the property of the United States Government and holders are required to safeguard their ID Card/CAC at all times. Pursuant to Directive Type Memorandum (DTM) 08-003 requires an individual to present documentation from the Supervisor or

LOST OR STOLEN CACs (continued)
Counseling form for military members, or lost CAC memorandum from an authorized official for DA Civilians, Contractors, and Foreign military members. This document MUST be scanned and stored into the DEERS database.

LOST/STOLEN ID CARDS FOR FAMILY MEMBERS
Dependents must have two (2) forms of Identification, please see General ID Instructions above. Sponsor MUST be present at time of appointment if a Power of Attorney or a valid DD Form 1172-2 is not available. Lost or Stolen memorandum not required.

POWER OF ATTORNEY
If your Military Sponsor is unavailable to accompany you for your appointment, a valid "General" or "Special" Power of Attorney (POA) will allow you to conduct business on your sponsor's behalf. The POA must be the original--no copies.

JUST GOT MARRIED? INITIAL ENROLLMENT OF SPOUSE
When adding your spouse to DEERS, the sponsor must be present or new spouse must have a valid POA with the following documents: a valid UNEXPIRED state or federal government issued photo ID, original/certified copy of marriage certificate,

original/certified birth certificate, Social Security Card/Verification of Social Security Number Memo from SSA Office with Social Security Number printed. If dual military, spouse MUST be enrolled and listed under both military personnel.

NAME CHANGE FOR SPOUSES
According to Defense Human Resource Activity (DHRA), the name may only be changed if the spouse provides the new Social Security Card reflecting the new name.

DEPENDENT I.D. CARDS
See above general ID instructions. Your sponsor must be present. If not available, the dependent must have a valid DD Form 1172-2 from their sponsor or a Power Of Attorney.

ADDING NEWBORN/CHILDREN TO DEERS
Sponsor must have the following documents for the child: Birth Certificate/Certificate of Live Birth authenticated by the attending physician & social security card (when available). If a stepchild is being added, the marriage certificate is also needed.

CHILDREN BORN OUT OF WEDLOCK
Female Sponsors:Child's Birth Certificate and Social Security Card; Male sponsors:Court order establishing paternity or a State Voluntary Acknowledgement of Paternity Form and child's Birth Certificate, and Social Security Card.

ENROLLING DEPENDENT WARD
DFAS Approval Letter and/or Court Document placing the child in the members household for at least 12 consecutive months, Birth Certificate and Social Security card. Army Sponsors do not require a DFAS approval to add a non-biological child.

FULL TIME COLLEGE STUDENTS AGES 21-23
If enrolled in DEERS or needs to replace a lost ID card, a letter from the school registrar certifying full-time course of study leading to an associate's degree or higher and anticipated graduation date from an ACCREDITED INST.

FULL TIME COLLEGE STUDENTS (CONTD)
If dependent turns 21 yrs old during the summer or winter breaks or loses their ID card, a letter from the registrar's office must be provided reflecting full time status in the previous semester before turning 21 and the following semester.

PARENT ENROLLMENT
Provide the letter of approval from DFAS at the time of enrollment, Service Members Birth Certificate & a valid Photo ID of your parent along with their Social Security Card and Birth Certificate.

PARENT-IN-LAW ENROLLMENT
Enrollment process and documents are the same as the Parent Enrollment listed but instead of Sponsor's Birth Certificate you will need SPOUSE'S Birth Certificate and Marriage Certificate. Note: All foreign documents must be translated and certified.

FOREIGN DOCUMENTS
Foreign documents must be accompanied with a certified English translation and an Apostille (If country is part of the 1961 Hague Convention *See Apostille)or the appropriate Certificate of Authentication by a US Consular Officer in the country of issuanc

Documents destined for use in participating countries and their territories should be certified by one of the officials in the jurisdiction in which the document has been executed.

RETIREMENT
For initial retirement, hardcopy of your DD Form 214-Member 4 Copy and two (2) forms of ID. Renewal or lost retired ID will also need two (2) forms of ID.

AGENT LETTER
Only Authorized Agents assisting Sponsors will schedule an appointment and indicate the Personnel Type as “OTHER”.

FORMER SPOUSE APPLICATIONS
Former spouse's MUST submit the following to apply: Divorce decree, marriage certificate, two forms of ID, Sponsor's DD Form 214/Statement of Service. This is an application and approval of application is up to the appropriate branch of service.

RAPIDS SELF SERVICE
For Sponsors that need to print a DD Form 1172-2 for housing or to authorize an ID Card Replacement(DD Form 1172-2 must not state "not for ID card issuance"), please Google "RAPIDS Self Service" and select the first link.

You may also nominate a family member to your DEERS record, and update family member's mailing address and phone numbers.

DISABLED AMERICAN VETERANS (DAV)
First time enrollment requires: Letter from the VA stating authorized to Commisary and Exchange privileges, DD Form 214 reflecting Honorable discharge, unexpired photo ID, Social Security Card and birth certificate.
