82ND AIRBORNE DIVISION

FORT BRAGG SOLDIERS
Check with your Brigade S1 to verify the location of support for your Unit. ALL Soldiers assigned to a Unit with CAC/ID Card capabilities MUST be serviced by their Unit.

MULTIPLE APPOINTMENTS
Individual appointments are required for EACH person requiring service or any other DEERS transaction

APPOINTMENT INSTRUCTIONS
For online appointments, click the date and then the time you would like for the appointment. If the calendar is not highlighted in GREEN, we do not have availability on that date. You may also call the appointment line at (910) 643-2737 to schedule.

IDENTIFICATION REQUIREMENTS
It is mandatory that ALL customers who are age 18 and older MUST have 2 unexpired forms of ID and the name MUST match. One MUST be a valid state or federal government issued picture ID.

Expired forms of identification are NOT ACCEPTED as a valid ID. See https://www.cac.mil/Portals/53/List_of_Acceptable_Documents.pdf?ver=2019-08-20-130159-397 for the List of Acceptable Identity Documents for DOD ID Card Issuance (FIPS 201-2).

ELIGIBILTY DOCUMENTATION
ALL eligibility documentation MUST be original or original certified true copies. Documents in any other form will NOT be accepted.

SPONSOR RESPONSIBILITIES
The sponsor MUST be present EVERYTIME a dependent is serviced. If you are unable to accompany your dependent, they MUST bring a Special DEERS Power of Attorney or General Power of Attorney to conduct business on the sponsor's behalf.

The DEERS POA MUST be in its original form. Photocopies are NOT accepted unless it is a certified true copy.

Visit your local DEERS/ID Card Facility to sign a DD 1172-2 giving your dependent authorization to receive an ID card in your absence. The form is valid 90 days from the date of your signature.

CAC PIN RESET/UNLOCK
No appointment is needed for a CAC PIN reset/unlock. The member MUST be present to reset/unlock the CAC.

RENEWAL
ID cards can be renewed within 90 days of expiration. Two unexpired forms of ID are required. (Check Identification Requirements for a list of the acceptable documents)

UNREMARRIED WIDOW/WIDOWER
INITIAL – Death certificate, Medicare Part B card if eligible. Two unexpired forms of ID are required. (Check Identification Requirements for a list of the acceptable documents)

INITIAL RETIREMENT
**Retirees and family members are not authorized to receive retiree ID cards until the official date of retirement. If the date falls on the weekend or holiday, the card can be issued the last business day prior to the date of retirement.**

Active Duty retirees MUST provide retirement orders and DD 214 (Copy 4).

Reserve/National Guard “gray area” retirees MUST provide orders/letter transferring them to the US Army Retired List and authorizing pay, and Medicare Part B card if eligible.

Medical retirees MUST provide retirement orders and DD 214 (Copy 4).

LOST/STOLEN/CONFISCATED ID CARDS
Two unexpired forms of ID are required. (Check Identification Requirements for a list of the acceptable documents) See below for additional specific instructions.

Servicemembers MUST provide a counseling statement digitally signed by the Unit commander or 1SG before issuance of a replacement CAC.

Civilian employees are required to provide a memorandum from their supervisor describing the date, place, and circumstances of the loss or theft.

Contractors are required to provide a memorandum from their COR, TASS agent, or supervisor describing the date, place, and circumstances of the loss or theft.

Dependents and retirees are required to provide a stamped sworn statement obtained from the Provost Marshal located at Bldg 2-5634 Armistead St, (910) 396-0391. The statement is valid for 10 days from the date of the reported incident.

DUAL MILITARY
Active Duty member married to another Active Duty member – marriage license, copy of spouse social security card, copy of spouse CAC front and back, divorce decree or annulment from former spouse if applicable

DIVORCE
Certified copy of the final dissolution of marriage

NAME CHANGE
Two unexpired forms of ID are required showing name change (Check Identification Requirements for a list of the acceptable documents)

SERVICEMEMBER - Military personnel record needs to be updated by the respective Service’s personnel data system. Memorandum showing personnel records have been updated and reflect name change, marriage certificate, divorce decree or annulment.

CIVILIAN EMPLOYEE – MUST be processed through CPAC prior to CAC update

CONTRACTOR – MUST be processed through your TASS agent prior to CAC update

SPOUSE/RETIREES/DAV - Source document for name change (i.e; final marriage license, divorce decree, annulment, court ordered name change)

CHILDREN – New birth certificate, new social security card, source document for name change (i.e; final adoption decree, court ordered name change)

FORMER SPOUSE
**ALL initial requests MUST be completed by the original branch of service. Renewals may be completed at any DEERS/ID Card Facility.**

Complete set of DD 214 (Copy 4), marriage license, divorce decree, two unexpired forms of ID are required showing name change (Check Identification Requirements for a list of the acceptable documents). ALL documents are needed to submit request.

DISABLED VETERANS (DAV)
Award letter stating member is 100% service connected disabled with commissary and exchange privileges, show future exam date, or unemployable, DD 214 (Copy 4) showing Honorable discharge

Two unexpired forms of ID are required (Check Identification Requirements for a list of the acceptable documents)

ADDING FAMILY MEMBERS
**Foreign documents NOT in English MUST be translated via certified translator or Military OneSource (for Active Duty members) at 1-800-342-9647. All foreign documents MUST have an Apostille or Legislation form the Embassy signed by a U.S. Consular.

The original document from the country and the certified translation along with the Apostille or Legislation is needed for enrollments.**

Foreign spouses and children may be enrolled and issued a 90 day temporary ID if they have applied but not received a social security number (SSN) or taxpayer identification number (TIN).

If SSN/TIN not received by ID card expiration, another 90 temporary card will be issued.

SPOUSE - The Sponsor MUST provide marriage license, birth certificate, social security card, and unexpired photo ID. (Check Identification Requirements for a list of the acceptable documents)

If applicable, Medicare Part B card, divorce decree, annulment, or death certificate of previous spouse

NEWBORN – Birth Certificate within 30 days of birth, social security card once received

**Male Sponsors adding children born out of wedlock need a court order establishing paternity or Voluntary Acknowledgement of Parentage from the state of birth.**

CHILD - Birth Certificate listing at least one parent, social security card

CHILD BORN OVERSEAS – Consular Birth Abroad (FS 240) or Certification of Birth (DS Form 1350) and social security card

ADOPTION – Final adoption decree or documentation from authorized placement under Department of Human Services or new birth certificate (listing at least one parent), new social security card

WARD (ARMY) – Court order granting custody to the Sponsor stipulating the child will be in the care and custody of the Sponsor for no less than 12 consecutive months, birth certificate, social security card

WARD (OTHER BRANCHES) – The ID card facility does not process the approval or provide any guidance for these requests. Initial requests and enrollment MUST be initiated through the parent branch.

Ages 21-23 – Letter within 30 days from the date from accredited institution or National Student Clearinghouse (select “User Login”), certifying full time status for Associate’s Degree or higher with anticipated graduation date

Sponsor MUST provide over 50% support, two unexpired forms of ID are required. (Check Identification Requirements for a list of the acceptable documents)

Students on break MUST provide enrollment verifications for the semester before and after to include graduation date, term dates, degree and full time status for each semester.

DFAS SECONDARY DEPENDENCY
The ID card facility DOES NOT process the approval or provide any guidance for these requests.

**DEPENDENCY REDETERMINATIONS MUST BE RECERTIFIFED EVERY 4 YEARS OR IF STATUS OF THE SPONSOR CHANGES WHICHEVER IS SOONER**

INCAPACITATED CHILD – DFAS approval letter, medical sufficiency statement from physician dated within 90 days of application, social security card, if 18 and older; unexpired form of photo ID are required.

PARENTS - DFAS approval letter, social security card, sponsor’s birth certificate, two unexpired forms of ID are required.

PARENT-IN-LAW - DFAS approval letter, social security card, sponsor’s birth certificate and marriage license, two unexpired forms of ID are required.

TRICARE YOUNG ADULT (TYA), 21-26
Premium-based healthcare plan for qualifying adult children between the ages of 21-26 that includes medical and pharmacy benefits only. (visit www.tricare.mil)

ID cards cannot be issued prior to the effective date of the healthcare plan. Sponsor MUST be present, provide POA, sign DD 1172-2 or with CAC access visit https://milconnect.dmdc.osd.mil/milconnect/.

TRICARE FOR LIFE (TFL)
Turning 65? Sign up for Medicare PART B benefits prior to your birthday to avoid a break in coverage.

Within 60 days from your birthday visit your local DEERS/ID card facility to update your record to reflect TFL.
