Hours of Operation:
Monday through Friday 0730-1130 & 1230-1600. On the first Thursday of each month, the ID Card Facility is closed from 0730-1230.
RENEW CAC AND DEPENDENT ID CARDS
Renewal of ID Cards can be completed no earlier than 90 days before the ID card expires.

TYPES OF ID
Our office ONLY issues ID's for Service Members (including National Guard/Reserve), Retired Service Members and their dependents, DoD Civilian, DoD CIV Retired, 100% DAV, DoD Contractor EMPLOYEE's ONLY.

APPOINTMENT INSTRUCTIONS
A scheduled appointment timeframe is 15 minutes per appointment. A session may consist of (1) CAC ID or up to (2) Dependent ID cards. Please schedule multiple consecutive appointments based on the number of ID cards needed for your Family.

**LATE TO YOUR APPOINTMENT**
Please keep your appointment time. If you arrive 5 minutes after your appointment time, you will need to reschedule.

GENERAL ID INSTRUCTIONS
All customers age 18 and older must present (2) forms of identity documents. One must be a valid unexpired state or federal government issued picture ID. See website for approved ID's http://www.dmdc.osd.mil/rsl/html/RequiredDocuments.html

UPDATE ADDRESS & PHONE # IN DEERS
Updates to your DEERS record require an appointment. Please visit the following website and update your information: https://www.dmdc.osd.mil/milconnect.

CAC RESET: INVALID PIN/BLOCKED/LOCKED ONLY
No appointment is needed for a CAC PIN reset ONLY if you received an error stating you have been "BLOCKED" or "LOCKED" or you have an "INVALID PIN". This action will be conducted as a Walk-In service, however, scheduled appointments have priority.

LOST OR STOLEN CAC/ID CARDS
Individual is required to present documentation from the Supervisor or Security officer confirming that their ID/CAC has been reported Lost or Stolen. This document MUST be scanned and stored into the DEERS database. *2 forms of ID are still required*.

LOST/STOLEN ID CARDS FOR FAMILY MEMBERS
Dependents must have two (2) forms of Identification, please see General ID Instructions above. Sponsor MUST be present at time of appointment if a Power of Attorney or a signed DD Form 1172-2 is not available. Lost or Stolen memorandum not required.

POWER OF ATTORNEY
If your Military Sponsor is unavailable to accompany you for your appointment, a valid "General" or "Special" Power of Attorney (POA) will allow you to conduct business on your sponsor's behalf. The POA must be the original--no copies.

INITIAL ENROLLMENT OF SPOUSE
When adding your spouse to DEERS, the sponsor must be present or new spouse must have a valid POA with the following documents: a valid UNEXPIRED state or federal government issued photo ID,

INITIAL ENROLLMENT OF SPOUSE (CONT'D)
original/certified copy of marriage certificate, original/certified birth certificate, Social Security Card/Verification of Social Security Number Memo from SSA Office with Social Security Number printed. If dual military, spouses MUST be enrolled in both

NAME CHANGE FOR SPOUSES
According to Defense Human Resource Activity (DHRA), the name may only be changed if the spouse provides the old Social Security Card along with the letter from SSN office, or the new Social Security Card reflecting the new name.

DEPENDENT ID CARDS
See above general ID instructions. Your sponsor must be present. If not available, the dependent must have a DD Form 1172-2 Application signed by their sponsor witnessed by a Verifying Official, or a POA.

ADDING NEWBORN/CHILDREN TO DEERS
Sponsor must have the following documents for the child: Birth Certificate/Certificate of Live Birth authenticated by the attending physician & social security card (when available). If a stepchild is being added, the marriage certificate is also needed.

FULL TIME COLLEGE STUDENTS
College students 21yrs old must have a letter from their school registrar's office stating that they are currently enrolled, full time with an expected graduation date in an accredited college in pursuit of an AS Degree or higher to have benefits extended

CHILDREN BORN OUT OF WEDLOCK
Female Sponsors: Child's Birth Certificate and Social Security Card; Male sponsors: Court order establishing paternity or a State Voluntary Acknowledgement of Paternity Form and child's Birth Certificate, and Social Security Card.

ENROLLING DEPENDENT WARD
You will need to provide a Court Document placing the child in the members household for at least 12 consecutive months, Birth Certificate and Social Security card. Please note that all documents must be original.

PARENT ENROLLMENT
Provide the letter of approval from DFAS at the time of enrollment, Service Members Birth Certificate & a valid Photo ID of your parent along with their Social Security Card and Birth Certificate.

PARENT-IN-LAW ENROLLMENT
Enrollment process and documents are the same as the Parent Enrollment listed but instead of Sponsor's Birth Certificate you will need SPOUSE'S Birth Certificate and Marriage Certificate. Note: All foreign documents must be translated and certified.

RETIREMENT
For initial retirement you will need DD Form 214 Member 4 Copy and (2) forms of ID. Primary does need to be unexpired photo ID. Renewal or lost retired ID will also need (2) forms of ID.

CAC Replacement due to technical issues
If you are experiencing email or website issues, please contact your IT personnel. If you make an appointment to renew your CAC and it is deemed to be functional by this office through verification of certificates and CHUID, we will not replace the CAC.
