SPONSOR MUST BE PRESENT FOR ALL TRANSACTIONS:
The sponsor is the person affiliated with the DoD/uniformed Services who takes responsibility for verifying and authorizing the applicant’s need for an ID card. A valid Power of Attorney or pre-sign 1172-2 can be used if the sponsor is not available.

PER EXORD 189-13: ALL SOLDIERS FROM THESE BRIGADES MUST GO TO THE BRIGADE S1 FOR ALL CAC ISSUES. IF YOUR BRIGADE CANNOT HELP YOU THE NCOIC MUST CALL 772-6423 TO MAKE ARRAGMENTS. APPOINTMENTS MADE AT CLARK HALL WILL BE CANCELED.

YOU MUST MAKE AN APPOINTMENT:
Our office is 100% appointment based. An appointment must be made for each individual. If you come to DEERS without an appointment, we will offer you the next available appointment, which may not be the same day. Emergencies don't need an appointment.

DEPENDENT ID CARD RENEWAL:
What you need: Sponsor-unexpired photo ID and a second form of ID for anyone age 18 and over. Old ID card may be used if no status change. Expired drivers license cannot be used even with military extension. ID's can be renewed 90 days prior to expiration

LOST/STOLEN CAC CARDs:
What you need: an unexpired state or government issued photo ID and a second form of ID. A DHR form 506E; signed by E6 or above for Military, GS11 for Civilians and Supervisor for Contractors. E7 and above and GS11 and above sign for themselves.

SPOUSE ENROLLMENT:
Original or Certified copy of Marriage and Birth certificate, Social Security Card (Original, unexpired state or government photo ID (Original form), Divorce decree if married to a military person. If Spouse was a dependent, sponsors SSN.

FOREIGN SPOUSE ENROLLMENT:
Original or Certified copy of Marriage Certificate (must have translation, Apostile or Certificate of Authentication by a U.S. Consular Officer attached). Two (2) forms of ID One must be a unexpired state or government issued photo ID.

NEWBORN AND CHILD ENROLLMENT:
Original or Certified copy of Birth certificate, Certificate of Live Birth or Certificate of Birth Abroad, Social Security Card. Male sponsors not married to the mother will need an Acknowledgement of Paternity or court order establishing Paternity.

STEPCHILD ENROLLMENT:
Original or Certified copy of Birth certificate, Marriage certificate and Social Security Card (original). If enrolled under another Sponsor you will need that Sponsors Social Security Number.

SPONSOR NAME CHANGE (MILITARY)
Sponsor (MILITARY)The new name must be showing in DEERS for us to issue a new CAC. Two forms of ID with your new name.

SPONSOR NAME CHANGE (CIVILIAN/CONTRACTOR)
The new name must be showing in DEERS for us to issue a new CAC. Two forms of ID with your new name.

SPOUSE NAME CHANGE:
Initial enrollment: Original or certified copy of marriage certificate, two forms of ID one being an unexpired photo ID, social security card. If enrolled two forms of ID with your new name, one being an unexpired photo ID.

ID CARDS FOR CHILDREN:
Children receive ID's at age 10. EXCEPTION: Children of dual military, single parent, does not live with sponsor or looks older then 10, an ID card is issued. Age 10 to 18 no documents needed. 18 and over two forms of ID, one being an unexpired photo ID

FULL-TIME STUDENTS AGE 21-23:
Letter from the school Registrar stating student is enrolled full-time with an estimated graduation date. If there is no estimated graduation date the ID will go to the end of the current semester. Two(2) forms of ID, one being an unexpired photo ID.

RETIREES:
Retirement orders or DD214 and two forms of ID, one being an unexpired photo ID (expired ID's cannot be used) Reserve/National Guard: 20 year letter, retirement orders or proof of receiving retired pay. Two(2)forms of ID, (expired ID's cannot be used)

ALL CONTRACTORS:
Record created in Trusted Associate Sponsorship System(TASS) by your Trusted Agent (TA). AHRC form 1 from your supervisor.(original) two forms of ID one being an unexpired photo ID. If going overseas Letter of authorization (LOA) with CAAF checked.

DISABLED VETERAN (DAV):
Documentation from the VA stating 100% Disabled or individual is unemployable due to disability caused by service-connected injury or disease(even with a lesser percentage), DD214 and two forms of ID one being an unexpired photo ID.

TRANSITIONAL HEALTH CARE PLAN (TA-180):
DD214 and two forms of ID on being an unexpired photo ID. You must be off Active duty to receive your TA-180. If you are going to Reserves or National Guard they have to pick you up as a gain before your TA-180 will populate in DEERS.

SECONDARY DEPENDENTS:
Approval letter from DFAS. Birth certificate for parent/s and family member of the parent/s. Two forms of ID one being an unexpired photo ID. You can get a packet from the ID card office or on the DFAS website.

MEDICARE A&B/TRICARE FOR LIFE:
MEDICARE A&B card and two forms of ID one being an unexpired photo ID. We can update DEERS up to 90 days before your 65th birthday. Once you turn 65 your TRICARE for life begins. If you turn down MEDICARE part B you are not eligible for TRICARE for Life
