APPOINTMENTS
Our appointments occur every 20 minutes. Please arrive early for your appointment. As always, there is a chance your appointment may be delayed. We ask for your patience as all scheduled customers will be seen.

APPOINTMENTS CONTINUED
1 Appointment Block can be scheduled for 2 dependent cards or 1 CAC card. If you need more, you must find consecutive blocks available and schedule. DO NOT BOOK EACH MEMBER OF YOUR FAMILY A SEPARATE APPOINTMENT BLOCK.

LATE POLICY
If you are late for your appointment, you have the option either to reschedule or wait until the next available time slot open to be seen.

IDENTIFICATION
All customers who are age 18 and older must have two (unexpired) forms of ID to get a new ID card made. One of them must be a state or government issued photo ID. This applies to all customers.

LOST/STOLEN I.D. CARDS
If your ID Card has been lost or stolen, you must obtain a Lost/Stolen CAC Memorandum from the Site Security Manager (YN2 Amanda Carter) or Police Department prior to your appointment.

ADDING A SPOUSE TO DEERS
When adding a spouse to DEERS, the sponsor needs to bring the following documents for their spouse: -Marriage Certificate, birth certificate, social security card, & a photo I.D. All documents must be originals or certified copies.

Walk-In Hours
The walk-in hours for our site are as follows: Monday and Wednesday 0800-0900 and Thursday 1100-1200. Please understand that we are only able to accept the first four customers based upon order of arrival.

DEPENDENT I.D. CARDS
For dependent ID cards, the sponsor must be present to sign the DD Form 1172. If not available, the dependent must have a previously signed 1172 (within 90 days of signature) signed by their sponsor and the VO/SSM or a POA. Call for further details.

DEPENDENT I.D. CARDS, CONT
When adding a child to DEERS, the sponsor must have the following documents for the child: birth certificate & social security card. If a stepchild is being added, the marriage certificate is also needed.

DEPENDENT I.D. CARDS, CONT
All full-time College students must bring a letter from their school registrar's office stating that they are enrolled full time in an accredited college in pursuit of an Associate Degree or higher and expected graduation date.
