The ID Card Section is closed on all Federal Holidays and Garrison Training Holidays.

CAC/ID Cards are the property of the US Government and holders are required to safeguard them at all times.

CAC/ID Cards may be renewed within 90 days of expiration.

APPOINTMENTS:
Reserving an appointment time will guarantee you will be seen ahead of the majority of walk-in customers. Appointment ONLY Number: (502)624-1526

You must schedule an appointment for EACH person requiring service. Only scheduled appointments will be seen.

If the calendar is not high-lighted in GREEN, we do not have any appointments on that date.

Because each transaction has different processing times, you may not be seen at your exact appointment time. Please keep this in mind when creating your appointment.

CANCEL/RESCHEDULE APPOINTMENT:
Customers will need their confirmation number to cancel/reschedule an appointment. Keep a copy of your appointment confirmation page. If more than five minutes late for your appointment, you will need to reschedule.

IDENTIFICATION:
ALL customers who are age 18 and older must have two VALID (unexpired) forms of ID to receive an ID card. One of them may be the old ID card (not expired) and Driver’s License.

If the ID card has expired the other form of documentation may be a Social Security Card or Birth Certificate.

DoD LIST OF ACCEPTABLE IDENTITY DOCUMENTS:
Applicants are required to provide two forms of identity source documents in original form. The identity source documents must be bound to the applicant and shall be neither expired nor cancelled.

If the two identity source documents bear different names, evidence of a formal name change must be provided. Log on to http://www.cac.mil/Portals/53/Documents/List_of_Acceptable_Documents.pdf to view the Acceptable Identity Documents.

LOST/STOLEN ID CARDS:
Active Duty members will need to provide a FK Form 5090 (http://www.knox.army.mil/Garrison/dhr/asd/forms.aspx) from their 1SG or Commander before reissuance of a replacement CAC. Member must also present two forms of valid ID.

Civilian employees will need to provide a FK Form 5090 (http://www.knox.army.mil/Garrison/dhr/asd/forms.aspx) from their Supervisor before reissuance of a replacement CAC and must present two valid forms of ID.

Contractors will need to provide a FK Form 5090 (http://www.knox.army.mil/Garrison/dhr/asd/forms.aspx) from their Contracting Representative (COR) before reissuance of a replacement CAC and must present two valid forms of ID

Retirees and dependents whose ID card was lost/stolen are required to fill out a FK Form 5090 (http://www.knox.army.mil/Garrison/dhr/asd/forms.aspx) at the ID Card Facility in order to receive a replacement ID card.

Two forms of valid ID must be presented for those who are over 18 years of age.

If your CAC/ID Card has been confiscated at the gate due to expiration, please provide the DES pass issued from the Visitor Control Center, you will need to provide the ID Card Section with two forms of unexpired ID to be issued a new CAC/ID.

ADDING A SPOUSE TO DEERS:
The Sponsor will need to provide the marriage certificate, birth certificate, social security card, and an unexpired photo ID. All documents must be original or certified copy.

All foreign documents must be original, the Apostle and the translated copy in English. Documents can be translated through Military One Source.

ADDING CHILDREN TO DEERS:
The sponsor must provide the child’s birth certificate and social security card. Documents must be original or certified copy.

Single male parents will need to provide the court order adjudicating paternity or affidavit of paternity from the state where the child resides, birth certificate and social security card.

ADDING STEP-CHILDREN TO DEERS:
The Sponsor will need to provide a marriage Certificate, child’s birth certificate and social security card. Documents must be original or certified copy.

WARDS:
Full court ordered guardianship of a child(ren) must be in the Sponsor’s name; Sponsor must provide the court order, birth certificate and social security card. Child(ren) MUST reside in the Sponsor household 12 months of the year to be authorized.

NEW BORN CHILD(REN):
The Sponsor will need to provide the Proof of Birth (hospital certificate or letterhead of hospital with parents and child names and signed by hospital administrator).

The Sponsor will need to provide DEERS with the original birth certificate and social security card when received.

Single male parents will need to provide the court order adjudicating paternity or affidavit of paternity from the state where the child resides, birth certificate and social security card.

FULL-TIME COLLEGE STUDENTS:
Students 21-23 years old; must be attending an accredited institution; provide a letter from the school registrar’s office certifying they are enrolled as a full-time student with an expected graduation date.

If no graduation date, the ID card will be issued semester to semester with updated documentation from the school.

Sponsor must provide over 50% support and the student needs to provide two forms of unexpired ID.

TRICARE FOR LIFE (TFL):
If you are turning 65:

To be eligible for Tri Care for Life (TFL) you should sign-up for Medicare benefits a few months before you turn 65 to avoid a break in coverage.

Once you receive your Medicare Card, within 60 days from your date of birth, you will need to bring the card into the nearest ID Card Section to have it updated in DEERS to reflect TFL.

You're eligible to enroll in Medicare Part B, at age 65 even if you are not eligible for Medicare Part A.

If your sponsor is on active duty, you may delay Part B enrollment without penalty.

Sign up for Part B when you are eligible to avoid a late enrollment penalty.

If you didn't sign up for Medicare Part B when you first became eligible, or you dropped it, you can sign up during Medicare's general enrollment period (January 1 - March 31 each year).

You may have to pay a Medicare Part B late enrollment penalty.

Your monthly premium for Part B may go up 10% for each full 12-month period that you could have had Part B, but did not sign up for it.

TRICARE YOUNG ADULT (TYA):
Dependent children between the ages of 21-26 and who are not full-time students, may apply for the Tri Care Young Adult (TYA). TYA is a premium-based health care plan that adult dependent children who qualify can purchase (www.tricare.mil).

ID Cards for TYA cannot be issued except on the date the health care plan starts.

NOTE: TYA is not authorized PX, Commissary or MWR. Dependent child(ren)must provide two forms of unexpired ID.

DIVORCE:
Certified copy of the Final Dissolution of Marriage.

REQUIREMENTS FOR DAV 100% DISABILITY:
DD 214, Letter from VA stating member has been rated 100% service connected, commissary/exchange privilege letter authorizing base privileges.

If adding a Spouse or child(ren), the Sponsor will need to provide a marriage certificate, social security card and birth certificate.

Two forms of valid unexpired ID will need to be provided for members over the age of 18.

INITIAL RETIREMENTS:
Retirement Orders or DD Form 214; must provide two forms of unexpired ID.

Reserve/National Guard "gray area" retirees must provide Orders/Letter transferring to the U.S. Army Retired List, authorizing retired pay and must provide two forms of unexpired ID.
